Every person in our care is treated with dignity. These are the rights we guarantee.

Every person in our care is treated with dignity and respect. In addition to the rights guaranteed by federal and state law, every Graceful Homes resident has the following rights.
Dignity and respect
- To be treated with consideration, respect, and full recognition of dignity and individuality.
- To be free from abuse, neglect, exploitation, and physical or chemical restraint used for discipline or convenience.
- To privacy in treatment, personal care, communications, and visits.
Care and self-determination
- To participate in developing and reviewing your own care plan and to be informed of your health status.
- To choose your own physician and to refuse treatment, and to be informed of the consequences of that choice.
- To manage your own financial affairs, or to a full accounting if you delegate them.
- To voice grievances without fear of retaliation, and to prompt efforts to resolve them.
Privacy and communication
- To confidential treatment of personal and medical records, and to approve or refuse their release except as required by law.
- To private, unrestricted communication and visits with people of your choice, including family, physicians, and legal representatives.
- To retain and use personal possessions as space permits.
Freedom of choice
- To participate in social, religious, and community activities of your choice, or to decline.
- To reasonable notice and due process regarding any transfer or discharge, and to an explanation of the reasons.
How to raise a concern
Residents and families may raise any concern at any time with our Care Director at (555) 014-2400, in writing to care@gracefulhomes.com, or anonymously through our family portal. You may also contact your state's Long-Term Care Ombudsman program and the state licensing agency. We will never retaliate against anyone for raising a concern.

Graceful Homes admits and cares for residents without regard to race, colour, national origin, religion, sex, sexual orientation, gender identity, age, or disability, in accordance with applicable civil-rights laws.
